This post explores the science and application of Extracorporeal Shockwave Therapy (ESWT), a cutting-edge, non-invasive treatment for chronic and acute musculoskeletal pain. We will discuss the two primary types of shockwave therapy: Radial Pressure Wave (RPW) therapy and Focused Shockwave (FSW) therapy. I’ll explain the distinct physiological mechanisms of each, detailing how they stimulate the body’s innate healing processes—a concept known as mechanotransduction. You’ll learn how these therapies trigger neovascularization, enhance blood flow, modulate inflammation, and stimulate stem cell migration to regenerate damaged tissue. We will discuss the clinical applications for conditions like plantar fasciitis, tennis elbow, and other tendinopathies, and explain why a combined approach often yields the most comprehensive results. Finally, I will touch on the importance of integrating these advanced modalities with foundational treatments, such as chiropractic care, to restore both tissue health and biomechanical function for holistic recovery.


The Science of Healing: What is Shockwave Therapy?

For years, we’ve understood that the body has a remarkable capacity to heal itself. However, when an injury becomes chronic, the body’s natural healing response can stall. The area develops scar tissue, blood flow becomes poor, and the brain essentially “gives up” on sending the necessary resources to fix the problem. This is where shockwave therapy comes in.

Shockwave therapy is not an electrical shock. It is a sophisticated, non-invasive technology that uses high-energy sound waves directed at a specific area of the body. Think of it as a way to “reawaken” the healing process. These powerful acoustic waves penetrate the skin and travel into the damaged tissue, creating a controlled, therapeutic micro-trauma. This process, known as mechanotransduction, converts the mechanical energy of the sound waves into a cascade of biochemical signals within the cells.

This signaling tells the brain, “There’s an injury here that needs attention!” In response, the body initiates a powerful, targeted healing response that includes:

  • Increased Blood Flow and Neovascularization: The sound waves stimulate the release of angiogenic factors, such as Vascular Endothelial Growth Factor (VEGF). This leads to the formation of new blood vessels, a process called neovascularization. Improved circulation is critical because it delivers oxygen and vital nutrients to the damaged area and removes metabolic waste products, thereby accelerating tissue repair (d’Agostino et al., 2008).
  • Stem Cell Migration and Proliferation: Shockwave therapy has been shown to trigger the migration of your body’s own mesenchymal stem cells to the treatment site. These are the body’s master repair cells, capable of differentiating into various tissue types—such as tendon, ligament, or bone—to rebuild and regenerate damaged structures.
  • Modulation of Inflammation: While acute inflammation is a necessary part of healing, chronic inflammation is destructive. Shockwave therapy helps reset this process, shifting the area from a state of chronic, non-productive inflammation to an acute, pro-healing inflammatory phase. This “restarts” the healing cascade from the beginning.
  • Stimulation of Collagen Production: Collagen is the primary protein component of all connective tissues, including tendons and ligaments. Shockwave therapy stimulates the cells responsible for producing collagen (fibroblasts), promoting the synthesis of strong, organized, and functional new tissue.
  • Dissolution of Calcifications: In chronic tendinopathies, calcium deposits can form within the tendon, causing pain and stiffness. The mechanical force of the shockwaves helps to break down these calcific fibroblasts.

From a clinical perspective, what I find most compelling is how shockwave therapy effectively turns a chronic condition back into an acute one. It creates a new, controlled healing opportunity for tissues that the body had long since walled off with scar tissue and pain. This allows us to guide the body through a proper healing sequence over a series of treatments.

Radial vs. Focused Shockwave: Two Tools for Comprehensive Healing

The technology behind shockwave therapy is not one-size-fits-all. Leading manufacturers, such as Storz Medical, a Swiss company considered the gold standard in the industry, have developed two distinct types of shockwave applicators: Radial Pressure Wave (RPW) and Focused Shockwave (FSW). Understanding the difference is key to creating a truly effective treatment plan.

Radial Pressure Wave (RPW) Therapy

  • Mechanism: In radial therapy, the highest energy concentration is at the skin surface. As the acoustic pressure wave travels into the body, it spreads out, or “fans out,” dissipating its energy along the way. It can effectively treat tissues up to approximately 6 centimeters deep. Imagine a ripple effect on a pond, spreading outwards from the point of impact.
  • Best For: This wider, more superficial energy distribution makes radial therapy exceptionally effective for treating large muscle groups and superficial tendons. It is perfect for addressing compensatory muscle tightness and trigger points that develop as a secondary response to a primary injury.
  • Analgesic Effect: A significant benefit of radial therapy is its immediate analgesic (pain-relieving) effect. The pressure waves overstimulate sensory nerve fibers, which can disrupt the transmission of pain signals to the brain, according to the “Gate Control Theory of Pain.” Patients often experience an immediate reduction in pain and a noticeable increase in their range of motion right after the first session.

Focused Shockwave (FSW) Therapy

  • Mechanism: True to its name, focused shockwave therapy is incredibly precise. The device generates sound waves that travel into the body and converge at a specific, pinpoint-accurate focal point deep within the tissue. Think of it like using a magnifying glass to focus sunlight onto a single point. This technology allows us to deliver a high concentration of energy at a precise depth of up to 12.5 centimeters.
  • Best For: This precision makes focused shockwave the ideal treatment for deep, specific pathologies. It is the go-to modality for targeting the exact origin of a tendon injury (tendinopathy), ligament damage, joint-related pain, and even non-healing bone fractures (non-unions). The focused energy can effectively break up scar tissue and adhesions at the core of the problem.
  • FDA Approval: The efficacy of focused shockwave therapy is well documented, and the Storz Medical Duolith SD1 T-Top Ultra device has received FDA approval to treat chronic plantar fasciitis, one of the most common and stubborn musculoskeletal complaints.

The Power of Combination Therapy: An Integrative Approach

As an integrative practitioner, I rarely see an injury in isolation. A patient with lateral epicondylitis (tennis elbow), for example, doesn’t just have pain at the bony prominence of their elbow. They almost invariably present with significant tightness, trigger points, and fascial restrictions throughout the muscles of their forearm and even their biceps. The body has been compensating for the primary injury, leading to this widespread muscular dysfunction.

This is why the optimal and most comprehensive treatment strategy involves a combination of radial and focused shockwave therapies.

  1. Start with Radial Therapy: We begin by using the OrthoPulse Ultra 100 radial device to treat the entire kinetic chain involved. For our tennis elbow patient, we would treat the entire length of the forearm extensors and flexors, as well as the biceps. This immediately relieves muscle tension, improves blood flow throughout the region, and provides a welcome analgesic effect. It prepares the entire area for healing.
  2. Follow with Focused Therapy: Next, we use the Duolith SD1 T-Top Ultra focused device to target the primary site of injury—the degenerated tendon tissue at the lateral epicondyle. We are now delivering a powerful, regenerative stimulus directly to the source of the problem, a location we’ve made more receptive by first treating the surrounding tissues.

This combined approach is synergistic. The radial therapy addresses the widespread compensatory patterns, while the focused therapy drives deep regeneration at the origin of the pathology. A typical combination session lasts about 10-15 minutes, with about 2,500-3,000 pulses delivered per modality.

What to Expect During and After Treatment

A common question is, “Will it hurt?” Shockwave is a diagnostic tool as well as a therapeutic one. We always start at a very low energy level and gradually increase it based on patient feedback. The goal is to keep you in a “therapeutically uncomfortable” range—typically a 5 or 6 on a 1-10 pain scale. You are in complete control, and we can adjust the energy levels instantly, directly from the handpiece. Interestingly, the most tender areas often correspond to the areas of greatest damage, confirming our diagnosis.

Immediately following the session, most patients report a significant decrease in pain and a remarkable improvement in their range of motion. This initial relief is largely due to the analgesic effect. It’s important to understand that some of the original pain may start to creep back in after about 72 hours. This is normal and expected. The real magic of shockwave therapy happens over a series of treatments.

We typically recommend a course of 4 to 6 sessions, spaced about a week apart. With each successive treatment, the pain-free interval becomes longer, and the returning symptoms become less intense. This is a clear sign that the tissue is progressively regenerating. Less pain comes back each time because the underlying tissue is actually healing, until eventually, the pain doesn’t return at all because the tissue has been fully restored.

The Role of Integrative Chiropractic Care

As a chiropractor, I am fundamentally concerned with the structure and function of the body, particularly the nervous system and the musculoskeletal frame. Shockwave therapy is a phenomenal tool for regenerating tissue, but for healing to be complete and lasting, we must also address the underlying biomechanical faults that may have contributed to the injury in the first place.

This is where integrative chiropractic care becomes essential.

  • Restoring Joint Mechanics: After using shockwave to heal a tendon, a chiropractic adjustment can restore proper motion to the adjacent joints. For our tennis elbow patient, this could mean adjusting the elbow, wrist, and even the cervical and thoracic spine to ensure the entire kinetic chain is moving freely and correctly. If the joint biomechanics aren’t corrected, the same aberrant stresses will be placed on the newly healed tendon, leading to re-injury.
  • Addressing Postural Imbalances: We assess and correct postural distortions that place undue strain on muscles and tendons. Myofascial release techniques, corrective exercises, and spinal adjustments work hand in hand with shockwave therapy to create a more resilient and balanced musculoskeletal system.
  • Nervous System Function: Chiropractic care optimizes nervous system function, the master controller of all bodily functions, including healing. By removing nerve interference through spinal adjustments, we ensure that the brain can effectively communicate with the healing tissues, supporting the regenerative processes initiated by shockwave therapy.

In my practice, we don’t just treat the “what” (the tendinopathy); we address the “why” (the biomechanical and postural dysfunction). Combining shockwave therapy for tissue regeneration with chiropractic care for functional restoration provides a truly holistic and powerful pathway to recovery, helping patients not just feel better, but get back to their lives stronger and more resilient than before.
