A Personal Journey from Frustration to Functional Medicine

As a healthcare provider, my greatest desire has always been to see my patients get better. For years, I practiced within the conventional medicine framework. After completing my fellowship, armed with the latest pharmaceutical knowledge from studies I participated in with companies like Lilly and Novo Nordisk, I was convinced I had the tools to conquer diabetes and hormonal disorders. I began my career as a board-certified endocrinologist, eager to make a difference.

However, after a couple of years, a frustrating pattern emerged. I remember one evening, sitting down and confiding in a colleague, “I don’t want to do this anymore.” My patients weren’t truly getting better. They would return for follow-up appointments, often sicker than before, their list of medications growing longer with each visit. I felt like I was managing symptoms, writing script after script without ever addressing the root cause of their illness. Despite 14 years of rigorous training, I felt I was failing them. Nobody was truly healing.

This professional crisis led me to the world of functional medicine. I attended training sessions, much like the ones I now lead, and began to see the body not as a collection of separate organs but as an interconnected system. The results were astounding. By addressing hormonal imbalances and lifestyle factors, my patients started losing weight, their energy returned, and we were able to start taking them off medications. It was a monumental shift.

Yet, even with this success, I noticed that about 25-30% of my patients still weren’t achieving the complete wellness they deserved. I was listening to their symptoms, running the right tests, and creating comprehensive plans, but something was still missing. That persistent “why?” that has driven me since I was a child pushed me to dig deeper. That’s when I had my second major epiphany: it all comes back to the gut. I can now say with absolute certainty that if you fix a person’s gut—and do it correctly—you are addressing the origin point of nearly all chronic disease.

The Microbiome: Your Body’s Inner Ecosystem

Over 2,000 years ago, Hippocrates, the father of medicine, famously stated, “All disease begins in the gut.” This ancient wisdom understood the centrality of our digestive system to overall health. Yet, somewhere along the path of modern medical advancement, we lost this fundamental truth. I find it staggering that in my extensive training as a board-certified endocrinologist with a degree in metabolism, I received only two lectures on the gut.

Your gut is home to the microbiome, a bustling community of trillions of microorganisms. Concentrated primarily in the large intestine, this inner ecosystem performs a staggering number of tasks vital for our well-being.

  • Digestion and Nutrient Absorption: The microbiome helps break down food components our bodies cannot digest on their own. More importantly, these microbes are essential for the absorption of vital nutrients. I see many patients who are taking hundreds of dollars’ worth of supplements and eating a pristine” organic” diet, yet their lab work shows significant nutrient deficiencies. The problem isn’t their intake; it’s their absorption.
  • Immune System Regulation: Approximately 70-80% of your immune system resides in your gut, within specialized tissue known as Gut-Associated Lymphoid Tissue (GALT). This makes your gut the primary gatekeeper and frontline defense against pathogens. Think about the “cold and flu season” that predictably arrives every January. This timing isn’t a coincidence. It follows a six-week period from Thanksgiving to New Year’s, during which we indulge in more sugar, processed foods, and alcohol. We are, in effect, assaulting our gut microbiome. When its defenses are compromised, we become vulnerable.
  • Hormone Conversion and Balance: The gut plays a critical role in regulating hormones. A specific collection of gut bacteria, known as the estrobolome, metabolizes estrogens. Gut health also affects the conversion of thyroid hormones (T4 to the active T3 form) and the regulation of key hormones such as insulin and cortisol.
  • Mood and Mental Health (The Gut-Brain Axis): Your gut produces a significant amount of neurotransmitters, including about 90% of your body’s serotonin, the “feel-good” chemical. An unhealthy gut can lead to imbalanced neurotransmitter production, contributing to anxiety, depression, and brain fog, a physical manifestation of the gut-brain axis.

Leaky Gut: The Gateway to Systemic Inflammation

When the intricate gut ecosystem is thrown off balance, we call it dysbiosis. This is a state where harmful bacteria, yeast, or parasites begin to outnumber beneficial bacteria. Dysbiosis is a key driver of a more serious condition known as Increased Intestinal Permeability, or “leaky gut.”

Our gut is lined by a single layer of specialized epithelial cells, tightly bound together by proteins called zonulin. In a healthy gut, this barrier acts like a meticulous gatekeeper, allowing digested nutrients into the bloodstream while keeping undigested food particles, toxins, and microbes safely contained. However, when zonulin signaling becomes dysfunctional—often due to chronic stress, inflammatory foods like gluten, toxins, or infections—those tight junctions break down. The epithelial cells separate, creating microscopic gaps in the gut lining.

This is where the real trouble begins. Substances that should never leave the gut—like undigested food proteins, artificial dyes, and bacterial endotoxins—”leak” into the bloodstream. Remember, 70% of your immune system resides just on the other side of this barrier. When it encounters these foreign invaders, it screams, “What is this? This doesn’t belong here!” and launches an aggressive, inflammatory response.

If your gut is leaky, this immune activation happens every time you eat. Your immune system releases a flood of inflammatory messengers called cytokines. This systemic inflammation has devastating, body-wide consequences: it elevates cortisol, increases insulin, blocks thyroid hormone conversion, and disrupts sex hormone levels. Essentially, every meal makes your body think it is under attack. This is why some of my patients, despite meticulously following a diet and exercise plan, still can’t lose weight or feel well. Until the underlying fire of a leaky gut is extinguished, they remain stuck in an inflammatory cycle.

The Gut-Hormone Connection: PCOS and Endometriosis

Nowhere is the impact of gut health more evident than in hormonal disorders like Polycystic Ovary Syndrome (PCOS) and endometriosis.

PCOS: An Inflammatory Cycle Driven by the Gut

In women with PCOS, we consistently see a state of gut dysbiosis, where harmful bacteria proliferate and produce inflammatory endotoxins called lipopolysaccharides (LPS). Here’s how LPS fuels the fire of PCOS:

  1. LPS Creates Leaky Gut: LPS acts like a drill, poking holes in the gut lining and causing increased intestinal permeability.
  2. Systemic Inflammation: As LPS and other inflammatory particles leak into the bloodstream, they trigger a body-wide inflammatory response.
  3. Insulin Resistance: This systemic inflammation is a primary driver of insulin resistance. The body’s cells become less responsive to insulin, prompting the pancreas to produce more insulin.
  4. Hyperandrogenism: High insulin levels directly stimulate the ovaries to produce excess androgens (like testosterone), leading to classic PCOS symptoms like cystic acne, hirsutism, and weight gain.

This creates a vicious cycle. Gut dysbiosis causes leaky gut, which drives inflammation and insulin resistance, which worsens hormonal imbalances. To prescribe metformin and tell a patient to “lose weight” without addressing the gut dysbiosis is missing the foundational piece of the puzzle.

Endometriosis and the Estrogen-Gut Axis

The gut’s influence extends deeply into estrogen balance, playing a critical role in conditions like endometriosis. This estrogen-dependent inflammatory condition is directly worsened by gut dysbiosis. Certain gut bacteria produce an enzyme called beta-glucuronidase, which effectively “reactivates” estrogen that the body has already packaged for elimination. This reactivated estrogen re-enters circulation, fueling the growth of endometrial lesions (Baker, 2017). It’s no surprise that women with endometriosis have a 50% increased risk of inflammatory bowel disease (IBD), highlighting this powerful link.

The Three Phases of Estrogen Metabolism: A Detoxification Deep Dive

To truly appreciate the gut’s role, you must understand how your body eliminates estrogen. This is a three-phase process involving the liver and the gut.

Phase 1: Hydroxylation (in the Liver)

The liver metabolizes estrogen down one of three main pathways:

  • 2-Hydroxyestrone (2-OH): The “good” and most protective pathway.
  • 16-Hydroxyestrone (16-OH): A more proliferative pathway associated with heavy bleeding and fibroids.
  • 4-Hydroxyestrone (4-OH): The “bad” or genotoxic pathway. These metabolites can damage DNA and have a strong link to breast cancer risk (Zahid, 2013).

Our goal is to promote the 2-OH pathway through nutrients such as Diindolylmethane (DIM) from cruciferous vegetables.

Phase 2: Conjugation (in the Liver)

Next, the liver “packages” these estrogen metabolites for excretion via methylation, a process that requires B vitamins and magnesium. Genetics, such as mutations in the MTHFR gene, is critical here. Individuals with MTHFR variants have impaired methylation, shunting more estrogen down the dangerous 4-OH pathway.

Phase 3: Excretion (in the Gut)

Once packaged, the estrogen metabolites are sent into the gut to be excreted in the stool. However, if you are constipated, the system breaks down. The stool, laden with toxic estrogens, sits in your colon. This allows harmful bacteria to produce beta-glucuronidase, which “unpacks” the estrogen. This newly freed, often-toxic estrogen is then reabsorbed into circulation through the leaky gut wall. This process, known as the enterohepatic recirculation of estrogen, creates a state of estrogen dominance.

The Gut-Thyroid Link: Unraveling Hashimoto’s

Hashimoto’s thyroiditis, the most common cause of hypothyroidism in the US, is an autoimmune condition where the body attacks its own thyroid gland. The trigger often starts in the gut.

Here is the functional medicine perspective:

  1. The thyroid uses iodine to make thyroid hormone. A natural byproduct of this reaction is hydrogen peroxide.
  2. In a healthy individual, the antioxidant enzyme glutathione peroxidase, which depends on the mineral selenium, neutralizes hydrogen peroxide.
  3. If a person is deficient in selenium, this byproduct builds up, causing oxidative stress that damages thyroid cells.
  4. When these cells break open, their contents spill out. The immune system mistakenly identifies these proteins as foreign invaders and mounts an attack, creating thyroid antibodies.

The problem isn’t necessarily too much iodine, as some claim. More often, it’s an inadequate supply of selenium and other antioxidants to clean up the natural byproducts of hormone production. This deficiency is often caused by a compromised gut that cannot properly absorb nutrients. It becomes a vicious cycle: leaky gut leads to poor nutrient absorption and inflammation, which in turn triggers the autoimmune attack. The resulting hypothyroidism then slows down gut motility, worsening the gut issues.

Key Nutraceuticals for Hormonal and Systemic Health

Targeted nutraceuticals can provide powerful support for these complex pathways.

The D3, K2, and A Trio for Immune and Bone Health

Vitamin D3 is a crucial hormone for immune regulation and calcium homeostasis. In my clinical experience, the connection between low Vitamin D and disease is undeniable. I once reviewed the first 100 cases of thyroid cancer I diagnosed and found that 99 of them were deficient in Vitamin D. I advocate for an optimal range of 60-80 ng/mL, far above the standard lab minimum.

However, Vitamin D needs partners. When you take D3, you absorb more calcium from your gut. Vitamin K2, produced by gut bacteria, acts as a traffic cop, activating proteins that direct calcium into your bones and teeth, keeping it out of your arteries where it could cause dangerous calcification (Vermeer, 2012). Vitamin A works alongside them to regulate bone remodeling. This D3-K2-A synergy is critical for skeletal, immune, and cardiovascular health.

Iodine: The Misunderstood Essential Mineral

Your thyroid needs iodine to produce hormones T4 and T3. A deficiency can lead to hypothyroidism. But iodine’s role extends beyond the thyroid. It is also concentrated in breast, prostate, and ovarian tissues, where it offers protective, anti-cancer properties. Iodine competes with toxic halogens like fluoride and bromide, which are prevalent in our environment. Sufficient iodine intake helps displace these toxins from your cells, protecting them from damage (Aceves et al., 2013).

DIM and Shilajit: Powerhouses for Hormone Modulation

Diindolylmethane (DIM) is a phytonutrient from cruciferous vegetables that is non-negotiable in my hormone protocols. It steers estrogen metabolism toward the protective 2-OH pathway and away from the harmful 4-OH and 16-OH routes. It also supports the function of the BRCA1 and BRCA2 tumor suppressor genes and has been shown to reduce dense, fibrous breast tissue.

Shilajit is an ancient substance rich in fulvic acid that excels at boosting free testosterone—the biologically active form. In my practice, patients on hormone therapy often experience a dip in energy toward the end of their cycle, even with good total testosterone levels. This is due to declining free testosterone. Shilajit counters this. A landmark study showed that 250 mg twice daily increased free testosterone by 51% (Pandit et al., 2016). A female patient of mine on pellet therapy saw her free testosterone nearly double, from 2.6 pg/mL to 4.6 pg/mL, after a year of using a supplement with Shilajit, reporting consistent energy throughout her cycle.

The Foundational Role of Integrative Chiropractic Care

While these nutritional strategies are powerful, they work best within a body that is structurally and neurologically sound. This is where integrative chiropractic care becomes an indispensable part of a holistic treatment plan.

Your digestive and endocrine systems are controlled by the autonomic nervous system (ANS). For optimal “rest-and-digest” and hormonal balance, your body needs to be in a parasympathetic state. In my clinic, I’ve observed that many patients with chronic digestive and hormonal issues also present with vertebral subluxations—misalignments in the spine, particularly in the thoracic and lumbar regions. These misalignments can interfere with the nerve signals traveling from the spine to the digestive and endocrine organs, keeping the body stuck in a sympathetic “stress” state.

Chiropractic adjustments work to correct these subluxations, restoring proper nerve flow. This has several direct benefits:

  • Improved Gut Motility: Proper nerve function helps regulate peristalsis, alleviating constipation or diarrhea.
  • Reduced Systemic Stress: By calming the sympathetic nervous system, chiropractic care helps lower cortisol and systemic inflammation, creating a better environment for healing.
  • Enhanced Vagal Tone: The vagus nerve is the superhighway of the parasympathetic system connecting the brain and gut. Adjustments, particularly in the upper cervical spine, can improve vagal tone, strengthening the gut-brain-hormone connection.

By integrating chiropractic care with functional medicine protocols, we address both the biochemical and neurological drivers of dysfunction, creating a powerful, synergistic effect that accelerates healing and provides lasting results.

A Warning on Supplement Quality

I must conclude with a critical warning. The supplement industry is notoriously under-regulated. Independent testing has found that an alarming percentage of products from major online retailers—sometimes as high as 70%—either do not contain the listed ingredients or are contaminated. This is why I am adamant that my patients obtain their supplements from a trusted, professional-grade source. Using low-quality supplements is not just a waste of money; it can be actively harmful and will sabotage our efforts to restore your health.
