Patient learning about ear points in auriculotherapy treatment.
This educational post explores battlefield acupuncture (BFA), an advanced auricular therapy technique that has gained significant traction in both clinical and emergency settings for its rapid, effective approach to pain management. Drawing on the expertise of leading researchers and clinicians, this post walks through the physiological rationale behind BFA, identifies the five key auricular acupuncture points, explains the use of ASP (Aiguille Semi-Permanente) needles, and outlines the step-by-step clinical protocol for administration. Additionally, this post highlights how Dr. Alex Jimenez, DC, APRN, FNP-BC, CFMP, IFMCP, ATN, CCST, integrates BFA within a broader multidisciplinary, integrative care model at Injury Medical Clinic PA in El Paso, Texas, alongside Dr. Maria Guadalupe Cardenas, MD, Board Certified in Internal Medicine, who serves as Medical Director and Collaborative Physician. Together, their team delivers a comprehensive, evidence-based approach to pain relief that bridges chiropractic care, internal medicine, functional medicine, and rehabilitation.
Battlefield acupuncture is one of the most compelling innovations in contemporary pain management, and its name alone tells a powerful story. Originally developed to provide rapid analgesia in austere environments, including military combat zones, this technique has since proven itself in emergency rooms, post-operative recovery suites, and outpatient clinical settings. The technique is rooted in auricular acupuncture, a system of medicine that maps the human body onto the external ear, or auricle, and uses needle stimulation at specific points to modulate pain and neurological function.
The physiological basis for auricular acupuncture is well established in the scientific literature. The ear is richly innervated by several cranial and cervical nerves, including branches of the vagus nerve (cranial nerve X), the auriculotemporal nerve, the lesser occipital nerve, and the greater auricular nerve. This dense innervation creates a direct neurological communication pathway to the central nervous system (CNS), particularly structures involved in pain perception and modulation such as the thalamus, cingulate gyrus, and the reticular activating system (Niemtzow, 2007). When ASP needles are placed at specific auricular points, they generate a continuous afferent signal that engages these central pain-processing centers, effectively downregulating pain perception through neuromodulation.
As I reflect on years of integrative clinical practice, I find battlefield acupuncture to be one of those techniques that bridges ancient therapeutic wisdom and modern neuroscience in a way that is both elegant and immediately practical.
The BFA protocol is built around five specific auricular acupuncture points, each selected for its neurological relevance to pain processing. Understanding these points requires an appreciation of both auricular anatomy and central nervous system physiology.
The five points, progressing from inferior to superior on the auricle, are:
Together, these five points create a comprehensive neuromodulatory circuit that addresses pain at multiple levels of the CNS, from the spinal cord relay stations to the cortical and subcortical centers that give pain its subjective, experiential quality.
The instrument used in battlefield acupuncture is the ASP needle, which stands for Aiguille Semi-Permanente. Unlike traditional acupuncture needles that are inserted and removed within a single session, ASP needles are designed to remain embedded in the auricular cartilage for five to seven days following insertion.
This semi-permanent design is critical to BFA’s clinical effectiveness. By maintaining a continuous low-level afferent neural signal at the auricular points over multiple days, the ASP needle sustains the neuromodulatory effects far beyond what a single treatment session could achieve. The needles are extremely small, typically 0.2 mm in diameter, and feature a small retention tip that anchors them safely within the ear cartilage.
From a materials standpoint, ASP needles are available in both gold and stainless steel, and some clinicians select the needle material based on the patient’s individual response or sensitivity. The insertion technique requires precision, and the three-finger technique commonly taught in BFA training ensures proper placement with direct, controlled pressure. The needles are typically well tolerated, causing minimal to no discomfort upon insertion, which makes this technique suitable even for pain-sensitive patients.
The BFA protocol is systematic and can be adapted based on patient response, which is one of its greatest clinical strengths. Here is how the procedure is performed in practice:
Ear Selection and Laterality
Point Identification and Needle Insertion
Post-Insertion Reassessment and Progressive Protocol
Treatment Frequency and Maintenance
At Injury Medical Clinic PA (also known as Mission Plaza Injury Medical Clinic) in El Paso, Texas, battlefield acupuncture is not practiced in isolation. It is one component of a deeply integrated, multidisciplinary care model that I, Dr. Alex Jimenez, DC, APRN, FNP-BC, CFMP, IFMCP, ATN, CCST, have developed over decades of clinical experience in both chiropractic and functional medicine practice (Jimenez, 2024).
A cornerstone of this integrative model is the partnership with Dr. Maria Guadalupe Cardenas, MD (NPI #1164426749, Texas MD License #J2933), Board Certified in Internal Medicine with over 40 years of experience as an internist. Dr. Cardenas serves as Medical Director and Collaborative Physician at the clinic, providing essential medical oversight to ensure every patient receives the highest standard of evidence-based care. This collaboration between an MD and a DC is a hallmark of integrative and injury care clinics, where complex patient presentations require perspectives from multiple disciplines working in concert.
This collaborative structure ensures that patients with complex pain conditions, whether arising from auto accidents, workplace injuries, sports trauma, or chronic degenerative conditions, receive care that is simultaneously comprehensive, coordinated, and compassionate (Personal Injury Doctor Group, 2024).
The synergy between chiropractic care and battlefield acupuncture is physiologically meaningful, not merely philosophical. Chiropractic adjustments, particularly high-velocity low-amplitude (HVLA) spinal manipulation, are known to modulate the autonomic nervous system, reduce central sensitization, and stimulate mechanoreceptors that compete with nociceptive (pain) signals in the dorsal horn of the spinal cord, a mechanism described by the gate control theory of pain (Melzack & Wall, 1965).
When combined with BFA, which operates at the level of the thalamus, cingulate gyrus, and limbic system, the two therapies address pain from fundamentally different levels of the nervous system simultaneously. Chiropractic care targets peripheral and spinal pain processing, while BFA engages supraspinal and cortical pain networks. Together, they create a multi-level neuromodulatory effect that is greater than either therapy could achieve alone.
Furthermore, by reducing the patient’s overall pain burden through BFA, chiropractic adjustments become better tolerated, more effective, and easily to sustain throughout a course of treatment, especially in acute injury presentations where guarding and muscle splinting may otherwise limit the clinician’s ability to perform effective spinal manipulation.
Research published in Medical Acupuncture (Niemtzow, 2007) and subsequent military and civilian studies have consistently demonstrated that BFA can produce rapid, significant reductions in pain scores, often within minutes of needle placement. Studies involving active-duty military personnel, emergency room patients, and post-operative patients have shown BFA to be a safe, effective, and pharmacologically complementary approach to acute and chronic pain management (Fedoruk & McLay, 2009).
In my own clinical observations documented through over two decades of integrative practice, patients presenting with cervicogenic headaches, lumbar radiculopathy, fibromyalgia, post-surgical pain, and musculoskeletal injury have demonstrated measurable improvements in pain scores, functional capacity, and quality of life when BFA was incorporated into their care plan alongside chiropractic manipulation and functional medicine interventions (Jimenez, 2024).
