Otherwise, you’re just treating a symptom, and you’ll continue to feel like crap because the foundational issue remains unaddressed.
Here are the most common culprits behind secondary hypogonadism that I see in my practice.
When a patient’s lab results point to secondary hypogonadism—low testosterone accompanied by low or inappropriately normal LH and FSH—our work as functional medicine detectives truly begins. Simply prescribing testosterone would be a disservice, akin to patching a hole in a dam without investigating the crack in its foundation. Suppression of the hypothalamic-pituitary-ovarian (HPO) axis signals a deeper systemic stressor at play. The body, in its infinite wisdom, is diverting resources away from functions it deems non-essential for immediate survival, such as reproduction and peak performance, to deal with a more pressing threat.
Our job is to identify that threat. The investigation typically follows a hierarchy of probabilities, starting with the most common and impactful culprits.
The very first place I look is the thyroid. The thyroid gland is the master conductor of the body’s metabolic orchestra. Its hormones, primarily thyroxine (T4) and triiodothyronine (T3), regulate the metabolic rate of every single cell in the body. The relationship between the thyroid and sex hormones is incredibly intimate and complex. When the thyroid is out of balance, it creates a domino effect that can directly sabotage testosterone levels.
The most common connection involves a protein called Sex Hormone-Binding Globulin (SHBG). SHBG is produced in the liver, and its primary job is to bind to sex hormones—testosterone, dihydrotestosterone (DHT), and estrogen—and transport them through the bloodstream. Think of SHBG as a fleet of taxi cabs for hormones. However, when a hormone is bound to SHBG, it is biologically inactive. It’s in the car, but it can’t get out to do its job at the cell receptor. Only the “free” portion of the hormone—the part not bound to SHBG—can exert its physiological effects.
Here’s where the thyroid comes in: Thyroid hormone levels directly regulate the liver’s production of SHBG.
The more common scenario I see causing low testosterone symptoms is an elevation in SHBG. A woman might have a total testosterone level that looks perfectly normal on a lab report. Her doctor might even pat her on the back and tell her everything is fine. But she still feels exhausted, has no libido, and can’t build muscle. Why? Because a large percentage of her testosterone is shackled to SHBG. Her free testosterone, the only part that matters, can be virtually zero. The SHBG is hoarding it all, rendering it useless.
This is why a standard thyroid panel that only checks Thyroid-Stimulating Hormone (TSH) is woefully inadequate. TSH is a pituitary hormone that tells the thyroid to work. It doesn’t tell us what the thyroid is actually doing or how the body is using the thyroid hormones. A comprehensive thyroid panel, which we run on every patient with hormonal symptoms, must include:
If we find that SHBG is elevated and the thyroid is the culprit (e.g., undiagnosed Hashimoto’s or an over-conversion of T4 to T3), the treatment isn’t testosterone. The treatment is to fix the thyroid. By restoring thyroid balance, SHBG levels normalize, freeing up the testosterone that’s already there and resolving the patient’s symptoms without ever needing a hormone prescription.
If the thyroid checks out, the next major area of investigation is the adrenal glands and the body’s stress response system, known as the Hypothalamic-Pituitary-Adrenal (HPA) axis. In our modern world, chronic, unrelenting stress—from work deadlines, financial worries, relationship troubles, poor sleep, or even perceived threats on social media—has become the norm. Our physiology, however, is still wired for acute, short-term threats, like being chased by a predator. It was not designed to handle a 24/7 onslaught of the stress hormone cortisol.
When you are chronically stressed out, the HPA axis goes into overdrive. The hypothalamus releases Corticotropin-Releasing Hormone (CRH), the pituitary releases Adrenocorticotropic Hormone (ACTH), and the adrenal glands pump out cortisol. This sustained cortisol production comes at a high cost to your sex hormones, a phenomenon known as the “pregnenolone steal” or, more accurately, the “cortisol shunt.”
To understand this, we need to look at the steroid hormone cascade. All steroid hormones—cortisol, DHEA, testosterone, estrogen, and progesterone—are synthesized from a common ancestor: cholesterol. The pathway looks something like this:

From pregnenolone, the pathway splits:
Under normal conditions, the body maintains a balance, directing pregnenolone down both pathways as needed. But when you are under chronic stress, the body’s survival instinct kicks in. The enzymatic machinery in the adrenal glands is “shunted” or “stolen” away from the sex hormone pathway and redirected toward the cortisol pathway.
The result? DHEA production, the direct precursor to testosterone in the adrenal glands, plummets. With less DHEA being made, there is less raw material available to produce testosterone. Cortisol production is prioritized, and sex hormone production is put on the back burner. This is a brilliant survival mechanism. In a true life-or-death situation, reproducing or building muscle is irrelevant. But when the “threat” is a looming deadline or a traffic jam, this chronic shunting leads to debilitating symptoms of low testosterone.
The solution here is painfully obvious, yet incredibly difficult for many to implement: you have to lower the HPA load. You cannot supplement your way out of a cortisol problem. You can’t take a magic tea or a peptide to override this fundamental survival mechanism. Giving testosterone might help symptoms in the short term, but it’s like putting a bucket under a leak in the roof. The real solution is to get on the roof and fix the shingles.
As part of our functional medicine approach, we work intensively with patients to identify their unique stressors and develop strategies to mitigate them. This is where chiropractic care can also play a direct role. Physical stress and pain are major contributors to the HPA axis load. Chronic back pain, neck pain, or headaches send a constant stream of nociceptive (pain) signals to the brain, which are interpreted as a threat, driving up cortisol. Chiropractic adjustments restore proper joint mechanics and motion, reducing this nociceptive input and helping down-regulate the sympathetic (fight-or-flight) nervous system. By alleviating physical pain, we reduce a major source of chronic stress, helping to rebalance the HPA axis and allowing the body to redirect resources back toward sex hormone production.

I’m seeing a particularly concerning trend lately, especially with the widespread use of GLP-1 agonist medications like Ozempic and Wegovy for weight loss. This trend is creating a severe, chronic energy deficit. Biology is ruthless in its logic: the body will never, ever invest in reproduction or peak performance when it perceives starvation.
The hypothalamus is exquisitely sensitive to energy availability. It has neurons that monitor circulating levels of glucose, fatty acids, and hormones like leptin (from fat cells) and insulin. When energy intake is drastically reduced, whether through an extreme diet, excessive exercise, or the appetite-suppressing effects of a GLP-1 medication, the hypothalamus receives a powerful danger signal. It interprets this lack of energy as a famine.
In response, it initiates a series of protective shutdowns to conserve energy for basic survival. One of the very first systems to go offline is the reproductive axis. The hypothalamus dramatically reduces pulsatile GnRH release. Without the rhythmic GnRH pulses, the pituitary stops releasing LH and FSH, and the entire HPO axis grinds to a halt. This is a condition known as Functional Hypothalamic Amenorrhea (FHA) in its most extreme form. Even less severe energy deficits can significantly suppress the axis, leading to secondary hypogonadism.
The body’s reasoning is simple: building a baby (reproduction) or building muscle (performance) are energy-expensive luxuries. In a famine, all available energy must be conserved for vital functions like keeping the heart beating and the brain working.
This is not just about weight loss. I see this in elite female athletes who are not adequately fueling their high training volume. I see it in women following extremely low-carb or ketogenic diets for prolonged periods without careful management. And I see it with increasing frequency in women using GLP-1 agonists who are eating as little as 800 calories a day. They may be losing weight, but they feel terrible. They are cold, their hair is falling out, they are exhausted, and their libido is gone—all classic signs of both hypothyroidism (as the body slows metabolism to conserve energy) and secondary hypogonadism.
The solution? You have to fix the energy issue. This can be a difficult conversation, especially for someone focused on a weight-loss goal. But the physiology is non-negotiable. We must find a sustainable balance with enough energy to support both health and a healthy body composition. This involves:
